About This Role
The Home Health RN Per Diem provides professional nursing care as a leader of the healthcare team, utilizing the nursing process in accordance with established standards of care, policies, and procedures. This role is ideal for those who are flexible and committed to delivering quality patient care.

What You'll Do
Uphold Kaiser Permanente's policies and procedures and applicable laws.
Serve as a leader of the healthcare team; delegate tasks appropriately.
Understand own and team members' scope of practice and escape as appropriate.
Demonstrate professional, supportive behavior and champion new ideas.
Lead and direct others through the change process.
Develop and/or contribute to the individualized plan of care.
Monitor patients' progress based on the care plan and revise as needed.
Ensure effective development and completion of discharge plans.
Ensure patient safety related to medications and procedures.
Provide an optimal patient care experience by actively listening to patients and families.
