Job description
Geisinger Home Health, a part of LHC Group within the Optum family, seeks a Registered Nurse to provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws and agency policies.
You will coordinate the total plan of care with other health care professionals, supervise home health aides and licensed nurses, perform required visits, document care accurately, and contribute to high-quality, continuous patient care across the care team.
