Job description
Home Health & Hospice Care is seeking a Registered Nurse for Greater Nashua NH area to deliver skilled, compassionate home care. You will perform comprehensive assessments (including OASIS), develop individualized care plans, and collaborate with physicians, families, and the interdisciplinary team to ensure high-quality, goal-directed care in patients' homes.
