Job description
Talented Medical Solutions seeks a Registered Nurse - Home Health to deliver skilled nursing care to patients in their homes, performing assessments, treatments and care coordination for an assigned caseload. Develops and updates the plan of care in collaboration with the physician and interdisciplinary team.
Educates patients and caregivers to support safe self-management and independence in the home setting, coordinating with therapy, social work, home health aides, and community resources.
