Job description
Supplemental Health Care is seeking a Home Health RN to provide skilled nursing care to patients in their homes. In this role, you will complete comprehensive assessments, develop and update care plans, and deliver treatments including wound care, IV therapy, and medication management. You will educate patients and families on disease management and safety, document care accurately in the EMR, and collaborate closely with physicians and the interdisciplinary team. Working independently in the field, you will manage your schedule, travel between patient homes, and uphold our mission-driven, people-focused culture while improving outcomes in the community.
