Home Health/PDN Clinical Supervisor Job Description

The Clinical Supervisor provides expertise and support for the provision of private duty nursing services, available at all times during operating hours. Using clinical knowledge and judgment, the Clinical Supervisor will supervise skilled nursing services and participate in all activities relevant to the professional services furnished, including the development of qualifications and the assignment of personnel.

Principal Duties and Responsibilities

The Clinical Supervisor is responsible for the direction, coordination, and supervision of services. The Clinical Supervisor's oversight must include the following:

Attend daily Team meetings, as schedule allows

Attend monthly Clinical Supervisor meetings

Communicate questions/concerns to leadership/management

Coordinate client care

Start of care visit/assessment (meet patient/family, collect documents, etc)

Recertification visits every 60 days

Build and maintain client chart and Care Plan in EMR

Additional visits/assessments after hospitalizations > 24 hours

Request signature on 485s, orders/medication requests

Delegate to office staff to follow up on faxes sent

Communicate with family/guardians during cert period regarding changes to care/new orders/appointment follow ups, as needed

Maintain monthly compliance with clients records at a score of 98% or higher

Manage Personnel:

Initial and annual skills verifications

Initial and annual Infection Control Observations & Evaluation

Initial and annual Statstaff updates

Per cert period: 'Supervisory Visit Report' form

Per week/month: 'Clinical Documentation Review' forms

PRN: Clinical support (answer clinical questions and concerns after hours for Full Time 'Exempt' CS)

Delegatable tasks (to office Support Staff)

Scheduling topics/needs

Questions regarding payroll or credentialing

Non-clinical related issues/communication

Other duties as needed or required

Qualifications & Skills

REQUIRED:

Current CA RN licensure.

A minimum of 2 years home care experience.

At least one year of supervisory experience.

Job Type: Part-time, expected hours 15-20 per week

Pay: $55 per hour
