Responsibilities:
Provide skilled speech-language pathology services to patients in their homes.
Conduct assessments and develop individualized care plans.
Collaborate with families and interdisciplinary teams to ensure optimal patient outcomes.
Manage caseload independently, documenting services per home health standards.
Educate patients and caregivers on speech, swallowing, and cognitive therapies.
