Job description
Optimal Care is seeking a Home Health Speech-Language Pathologist (SLP) to evaluate and treat communication and swallowing disorders in patients at home across a large service area in mid‑Michigan. You will conduct comprehensive assessments, design individualized therapy plans, and document progress under physician orders and plans of care.
Responsibilities include collaboration with the care team, education for patients and caregivers, and maintaining compliant clinical documentation.
