Job description
Family First Healthcare Services in Tampa, FL seeks a PRN Social Worker to provide psychosocial assessments, counseling, case management, and resource coordination for home health patients and families. You’ll collaborate with the IDT, support discharge planning, and connect clients with community resources, Medicaid waivers, transportation, and supportive services.
Valid MSW preferred; LMSW/LCSW acceptable with relevant experience.
