Job description
CenterWell Home Health seeks a Medical Social Worker to assess patients’ social and emotional needs, coordinate with the interdisciplinary team, and connect families with community resources in a home-health setting.
You will supervise the Social Worker Assistant, ensure compliance with company and regulatory standards, and document care progression and outcomes. Fluency in English and valid licensure are required.
