Job description
CenterWell Home Health, part of Humana Inc., seeks a Home Health Speech-Language Pathologist to provide in-home evaluation and treatment for seniors and adults with chronic conditions. In this role, you will assess speech, language, cognitive-communication, and swallowing disorders; develop and implement individualized care plans; educate patients and caregivers; and document in our electronic health record. You’ll collaborate closely with nurses, therapists, and physicians to support recovery, safety, and independence at home while contributing to a compassionate, patient-first care culture.
