n nQualifications of Home Health Speech Language Pathologist (SLP):
nnnCurrently licensed in the state(s) in which practicing
n
nnAt least 2 year or more of experience in home care
n
nnLevel 2 Background screening
n
n nResponsibilities and Duties of Home Health Speech Language Pathologist (SLP):
n· Improves or maximizes the communication of the patient.
n· Returns the individual to optimum and productive living within the patient's capabilities.
n· Periodically participates with all other health care personnel in patient care planning.
n· Provides full range Speech-Language Pathology Services as ordered by physician.
n· Takes initial history and makes initial evaluation.
n· Performs all skilled procedures.
n· Consults with physicians regarding change of treatment.
n· Writes reports to physicians regarding patient's progress.
n· Assists the physician in evaluating level of function.
n· Helps develop the plan of care and revises as necessary.
n· Prepares clinical and progress notes
n· Advises and consults with the family and other Agency personnel.
n· Participates in in-service programs.
n· Completes and submits assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency defined time frames
