Job description
Apollo Home Healthcare is seeking a Speech–Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa County, Alameda County, and the Tri-Valley area. This is a W-2 per diem role ideal for SLPs looking for schedule flexibility, autonomy, and meaningful patient relationships. You’ll provide high-quality, one-on-one care to patients in their homes while collaborating with a supportive interdisciplinary team.

nWhat You’ll Do
nAs a Home Health SLP, you will:
nnnPerform speech-language and swallowing evaluations in patients’ homes
n
nnDevelop and implement individualized Plans of Care
n
nnProvide skilled speech therapy interventions
n
nnRecommend communication devices and adaptive equipment as needed
n
nnAssess home environments for safety and functional communication needs
n
nnCollaborate with physicians, nurses, PT/OT, and care teams
n
nnParticipate in case conferences as appropriate
n
nnComplete timely, compliant clinical documentation
n
nnEducate patients and caregivers on exercises and compensatory strategies
n
nnSupervise home health aides when applicable to the plan of care
n
n nRequirements
nnnActive California Speech–Language Pathologist license
n
nnMaster’s degree in Speech-Language Pathology
n
nnCurrent CPR certification
n
nnHome health experience preferred (but not required)
n
nnStrong documentation and communication skills
n
nnAbility to work independently in the field
n
nnReliable transportation and valid driver’s license
