Job description
CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home or facility-based speech-language services under the supervision of the Clinical Manager. You will assess, diagnose, and treat communication and swallowing disorders to improve independence and quality of life for patients in their homes.
You will develop and review Plans of Treatment and Care, document progress, and participate in care coordination and discharge planning, while maintaining professional standards
