Job description
Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead and coordinate in-home nursing care for patients with life-limiting illnesses. You will assess, educate, and support patients and families, working with an interdisciplinary team to refine care plans and ensure compassionate, compliant care.
You will perform comprehensive assessments, deliver interventions, and document care while guiding families through end-of-life processes with empathy and professionalism.
