This position focuses on supporting patients' comfort and dignity by assisting with daily living activities, monitoring health status, and offering emotional support to both patients and their families. The CNA works closely with the interdisciplinary hospice team to ensure that care plans are effectively implemented and adjusted according to patient needs. This role requires a deep commitment to empathetic care, excellent communication skills, and the ability to work independently in a home setting. Ultimately, the CNA contributes significantly to enhancing the quality of life for patients during their end-of-life journey while respecting their wishes and cultural values.

Minimum Qualifications:

Current and valid Certified Nursing Assistant (CNA) certification in the state of Wisconsin.

High school diploma or equivalent.

Demonstrated experience in providing personal care to patients, preferably in a home health or hospice setting.

Collaborate with the hospice care team to implement individualized care plans and participate in care coordination activities.

Maintain a safe, clean, and comfortable environment in the patient’s home, adhering to infection control and safety protocols.

Educate family members on basic care techniques and provide guidance on how to support their loved ones effectively.

Skills:

The required skills enable the CNA to deliver high-quality, compassionate care by effectively assisting with daily living activities and monitoring patient health. Strong communication skills are essential for interacting empathetically with patients and families, as well as for collaborating with the hospice care team to ensure coordinated care. Attention to detail and documentation skills are used daily to accurately record patient status and care provided, which supports clinical decision-making.
