Job description
Visiting Nurse Association of Wisconsin, Inc., affiliated with Aurora Health Care, is seeking a compassionate Home Hospice RN Case Manager to provide skilled, patient-centered care in patients’ homes. Assess needs, create and update individualized plans of care, manage symptoms and medications, and coordinate services with physicians and the interdisciplinary hospice team. Educate and support patients, families, and caregivers through end-of-life care while promoting comfort, dignity, and respect for diverse communities.
