n
n nHome Infusion Clinical Liaison, RN Responsibilities include :
nnnFacilitates transitions of care from acute and subacute settings to home infusion and education of home infusion and enteral therapies to hospital and community providers.
n
nnAssigned to selected facilities to collaborate with providers, case managers, referral sources, and home health agencies to facilitate patient discharge and coordination of care.
n
nnCollaborates with business development team in gaining and maintaining market share through referral intake process.
n
nnResponsible for timely communication with all referral sources telephonically, through electronic platforms, and in-person.
n
nnResponsible for clear concise referral provided to pharmacy meeting all regulatory, payer, and safety requirements.
n
nnCompletes preadmission assessment and education to patient and caregiver of pharmacy services.
n
nnConducts the initial infusion and enteral set-up and patient/caregiver training on our equipment and therapy services.
n
nnInitiates formulary changes and recommendations on route of feeding and advances in nutrition via the direction of the pharmacy's registered dietitian.
n
nnResponsible for knowledge of Medicare and Managed Care regulations and requirements.
n
nnReviews the patient's medical record to obtain both pertinent medical history and primary/secondary insurance information and communicates this to pharmacy intake.
n
nnParticipates with any data collection required for therapy start and patient tracking processes, which includes facilitating the transfer of orders via phone, fax, and e-prescribing.
n
nnTimely response to referrals sources, providers, and leadership is essential.
n
nnMonitors and communicates referral source activity acting as one point of contact for referral sources, home health, and infusion.
n
n nThe Home Infusion Clinical Liaison, RN Requirements:
nnnBLS (Basic Life Support)
n
nnDriver's License - State of Florida
n
nnRN (Registered Nurse) License - State of Florida
n
nnRequired Associate's or Diploma Nursing
n
nnRequired 2 years of Nursing; And - 1 year of Infusion Related
n
nnRequired Business Development skills
n
nnRequires Reliable transportation- Must be able to travel in assigned territory and attend meetings as needed.
n
nnRequires infusion and enteral administration skills assessment
n
nnPreferred Computer proficiency in Microsoft Office and Excel
n
nnPreferred experience in Home Healthcare specialty Pharmacy
n
nnPreferred CRNI Certification
n
n nThis position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). More information can be found at .
