Job description
Pharmko is seeking an experienced Nurse Case Manager to join our clinical team in New York. This full-time, in-person role requires travel to hospitals and patient homes for bedside teaching and infusion education.
You will coordinate and monitor infusion patients, educate patients and caregivers, maintain precise EMR documentation, and ensure smooth hospital-to-home transitions while upholding clinical standards and regulatory compliance.
