Job description
Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home Medicare Advantage assessments. This is a non-prescribing, field-based role focused on education, gap closure, and collaboration with PCPs.
Responsibilities include comprehensive home visits, documenting diagnoses, and coordinating with primary care teams to improve health outcomes while offering a flexible schedule and no weekend/holiday requirements.
