Job description
Segue Health in Jackson, TN seeks an experienced Nurse Practitioner to join our faith-based home health team, providing in-home post-discharge care and medical treatment for patients returning home from hospital stays.
You will review discharge summaries, coordinate with home health professionals, and oversee ongoing care plans to promote recovery and reduce readmissions. Must hold TN NP license and Medicare provider number; willingness to travel within 35 miles.
