Job description
UnitedHealth Group's Optum Home & Community Care is seeking Advanced Practice Clinicians (Nurse Practitioners or Physician Assistants) to perform in-home health assessments for Medicare Advantage members in the Bronx. You will educate patients, close care gaps, and collaborate with PCPs to improve outcomes.
This role offers flexible schedules between 20-40 hours per week, with no on-call, weekends, or holidays.
