Job description
Summa Health seeks an IM or FM trained physician to join our Senior Health team, supporting home-based primary care in the community. The role contributes to Summa's Complex Care Institute and the interdisciplinary care plan for vulnerable patients.
The physician will join Summa Health Medical Group, a large multi-specialty practice with a strong academic model in a community teaching hospital. Epic EMR is used to coordinate care.
