Job description
Summa Health's Senior Health Team seeks an IM or FM trained physician to support home-based primary care in the community. The role advances Summa's Complex Care Institute, serving 500+ patients with integrated, cost-conscious care in a community setting.
The selected physician will join Summa Health Medical Group, a large multi-specialty practice with an academic care model in a community teaching hospital, using Epic EMR. Expect teaching roles and a Monday–Friday schedule with minimal call.
