Job description
Hcrhealth is seeking a skilled RN Case Manager to provide nursing care during home visits in Onondaga County, NY. You will coordinate patient care, educate families, and ensure compliance with physician orders.
Responsibilities include managing a full caseload, documenting progress, assessing home environments, and supervising Home Health Aides. Must hold a NYS RN license and have at least one year of home care experience.
