Job description
Your Health is seeking a Registered Nurse to serve wound care patients across the Savannah area. The Visiting Nurse Case Manager will visit patients at home and in care facilities, coordinating with providers and initiating telemedicine when appropriate.
Responsibilities include reviewing records, enrolling patients in Care Management, developing Plans of Care, coordinating with the care team, and documenting activities. Travel is required daily, with 10-hour shifts Monday-Thursday, 7am-6pm.
