Job description
Hartford Healthcare is seeking a compassionate HomeCare LPN to deliver skilled nursing care to patients in their homes across Connecticut. In this role, you’ll perform assessments, administer medications, manage wound and chronic disease care, and update electronic care plans in collaboration with RNs and providers. You’ll educate patients and families, coordinate services with the care team, and ensure safety and quality standards.
