n nResponsibilities
nn Accept and treat patients within your designated geographic area
n Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
n Develop a plan of care and treatment goals in consultation with the physician, patient, and family
n Plan, prepare, and implement customized therapy programs to restore function and promote independence
n Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
n Provide timely and accurate documentation for each visit
n Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
n Deliver care in compliance with agency policies and professional practice standards
n nnQualifications:
Qualifications
nn Graduate of an accredited Physical Therapy program
n Licensed Physical Therapist in the state of New York or eligible for licensure
n Home health experience is preferred but not required — we will provide you with a mentor to help with documentation and compliance
n Ability to travel to patients’ homes within the assigned geographic territory
n nAdditional Information
nThe anticipated base salary range for this position is $95,000–$110,000 annually . Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
nThis position is eligible for a $3,000 sign-on bonus. Additional terms and conditions may apply.
