Home Health Registered Nurse (RN) Travel Assignment in Barre, Vermont

Schedule: Monday-Friday with weekend rotation approximately every 4th week
Shift Duration: 12-hour rotating shifts
Weekly Estimated Pay: $2,130-$2,250
Guaranteed Hours: 36 per week
Openings: 3 positions available
Registered Nurse Requirements Active Registered Nurse (RN) license
Minimum 1 year of nursing experience required; home health or hospice experience preferred
Strong clinical assessment, care coordination, and patient education skills
Ability to work independently while collaborating with physicians and interdisciplinary team members
Experience with case management or lead discipline responsibilities is a plus
Ability to adapt to a rotating schedule and provide reliable weekend coverage as assigned
Home Health RN Responsibilities Deliver individualized nursing care using the nursing process in accordance with physician orders
Perform comprehensive patient assessments and develop appropriate care plans
Serve as case manager when acting as the lead discipline
Coordinate care across disciplines to support safe, effective patient outcomes
Educate patients and families on treatment plans, safety, and self-care
Document patient care accurately and maintain timely clinical records
Support a high standard of compassionate, community-based home health and hospice care
