Job description
RADIAS Health is seeking a Homeless Outreach Mental Health Practitioner to deliver PATH services to homeless individuals, focusing on unsheltered clients with mental health needs and co-occurring diagnoses. You will work within a multidisciplinary team and coordinate housing and recovery goals with clients, advocating for access to housing resources.
