Job description
Low T Center is seeking a Mid-Level Provider to join our Illinois clinics and support hormone replacement and targeted wellness programs. You will work under a physician medical director and the Center Director to coordinate patient care and ensure high-quality outcomes.
Qualified candidates must hold an active Illinois NP or PA license, a current DEA license, and a current Controlled Substance License, with the ability to float between Hoffman Estates, Burr Ridge and Westmont.
