Our service area covers Pueblo and surrounding areas.The ideal candidate will be compassionate, empathetic and able to dedicate themselves to our goal of providing the highest quality of care that focuses on the patient's physical, emotional and spiritual needs.Why Join Our Team?· We offer an environment that encourages team involvement and fosters new ideas· Excellent pay· Generous PTO Package· Employer 401k Matching with up to 4% Employer Match Available· Medical, dental, vision, along with the other ancillary benefits offered almost immediately (waiting period waived)CNA Job Summary: Provide care in the patient's place of residency in accordance with current federal, state, and local standards, guidelines, and regulations that govern our organization, and as may be required by the Director of Nursing or designee.CNA- Essential Job Functions:Demonstrates knowledge and adheres to the plan of carePerforms duties as assigned by Case Manager including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding and transferring patientCompetent taking and recording vital signs in the EMR, notifies appropriate personnel for values outside normal parametersDescribes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activitiesNotifies appropriate licensed personnel when patient complains of painPositions patient in bed with proper body alignmentAssists patient with ambulation as necessaryPerforms all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errorsWorks as part of the interdisciplinary teamPrepares clinical note within the specified time frame of the Agency (Documentation created on the day of the visit and completed within 24 hours)Apprehends the regulations for supervisory visits every 14 days and maintains complianceMaintains 12 hours of annual education with annual as required by state, federal, accrediting and agency regulationQualificationsRegulatory Requirements:CNA certificate obtained by: Completion of a CNA course Certified by State Department of Health ServicesOn-the-job instruction relative to specific patient needs and continuing in-service training, so that patient safety may be assured and the personal care and homemaking needs of the patient are adequately metAdheres to dress code, appearance is neat and cleanCompletes annual education requirementsMaintains regulatory requirementsMaintains patient confidentialityReports to work on time and as scheduled, completes work within designated timeCompletes in-services and returns in a timely fashionAttends annual review and department in-services, as scheduledRepresents the organization in a positive and professional mannerActively participates in performance improvement and continuous quality improvement activitiesComplies with all organizational policies regarding ethical business practicesCommunicates the mission, ethics and goals of the organizationDemonstrates effective time management and organizational skills; provides patient visits as scheduled; completes patient care within the designated time
