Job description
HopeHealth, Inc. in Providence, RI is seeking an experienced Registered Nurse (RN) for its Access Center.
This full-time role coordinates referrals and admissions to ensure timely hospice services, acting as a liaison among referral sources, care providers, staff, patients and families. This 40-hour-per-week position requires 2-3 years hospice experience, strong nursing assessment and communication skills, and the ability to schedule visits and resolve scheduling issues while supporting patients
