Job description
Chapters Health System is seeking a Hospice Access Physician to ensure timely access to hospice services. The role directs and educates admissions staff, facilitates eligibility determinations, and completes required regulatory documentation for each admission.
Ideal candidates hold an MD or DO, with Florida licensure and a willingness to obtain additional state licenses; BLS Certification is required, and breast hospice medicine certification is preferred.
