Job description
Hartford Healthcare seeks a compassionate Hospice Admission Nurse to coordinate and complete hospice admissions. In this role, you will conduct comprehensive patient assessments, explain hospice services, obtain consents, and develop initial care plans that honor patient and family goals. You will collaborate closely with physicians, interdisciplinary team members, and community partners to ensure safe, timely transitions to hospice care.
