Job description
Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to coordinate and complete hospice admissions for patients and families. In this role, you will conduct comprehensive assessments, verify eligibility, educate patients and caregivers on hospice services, and develop initial care plans in collaboration with the interdisciplinary team.
