Job description
Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to join our Healthcare & Medical Services team. In this role, you will complete comprehensive hospice admission assessments, educate patients and families on hospice services, and collaborate with an interdisciplinary team to develop individualized care plans. You will coordinate timely admissions from hospitals, clinics, and homes, ensure regulatory and documentation compliance, and support symptom management during the transition to hospice.
