Job description
Hartford Healthcare is seeking a compassionate Hospice Admission Nurse to coordinate and complete hospice admissions for patients and families. In this role, you will perform comprehensive assessments, explain hospice services, obtain consents, and develop initial plans of care in collaboration with the interdisciplinary team. You will educate patients and caregivers on symptom management, medications, and available resources while ensuring regulatory and documentation compliance.
