Job description
We are seeking an experienced Hospice Admissions Registered Nurse (RN) to evaluate and admit patients, develop individualized care plans, and coordinate patient services.
The role involves performing comprehensive assessments, documenting care at the point of service, and working independently with strong communication, organization, and time management skills.
Qualifications include: current RN licensure in the state, at least one year of clinical nursing experience, effective patient evaluation and documentation skills, and ability to work in a home setting.
Proficiency with computer applications (Windows, Word, Excel), CPR certification, a valid driver’s license, and a minimum of twelve hours of annual continuing education are required.
The position involves participating in on-call rotations and staff meetings, with travel and working conditions as assigned.
This role emphasizes delivering compassionate, dignified care to hospice patients, ensuring pain management and quality of life.
