Job description
Hartford Healthcare seeks a compassionate Hospice Admission Nurse to guide patients and families through the hospice admission process. In this role, you will perform timely in-home and facility-based assessments, verify eligibility, educate patients and caregivers on hospice services, and develop initial plans of care in collaboration with the interdisciplinary team.
