Job description
GBMC HealthCare is seeking a Hospice Liaison Nurse to generate admissions from hospital sources, coordinate clinical assessments, and foster relationships with Gilchrist service area hospitals. The role includes admissions from Mobile Hospice Response beds and ongoing case management during hospital stays.
The nurse will market services to referral sources, ensure HIPAA compliance, and collaborate with hospital staff on discharge planning to uphold GHC goals and quality care.
