Job description
GBMC HealthCare's Gilchrist Hospice Care seeks a Hospice Liaison Nurse to generate admissions from hospital referrals, coordinate assessments, and support discharge planning. The role requires an RN with Maryland license and at least three years of hospice, home health, critical care, ER or related experience, with strong communication and relationship-building skills.
Travel to area hospitals and community interaction are expected.
