Job description
Visiting Nurse Health System in Atlanta is seeking an Admissions Nurse for Hospice to coordinate care for a home-based caseload and ensure quality standards. You will assess, plan, implement, monitor, and evaluate comprehensive care plans to optimize patient outcomes.
The role requires leading patient education, collaborating with an interdisciplinary team, and supervising LPNs/HHAs while managing on-call duties. Georgia licensure and CPR are required.
