Job description
Addus Family Company in Albuquerque seeks a compassionate Hospice Admissions RN to coordinate admissions for patients in their homes. In this field-based role, you will conduct initial assessments, verify eligibility, obtain consents, and create individualized plans of care with physicians and the interdisciplinary team.
