What You'll Do
Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
Complete comprehensive clinical eligibility assessments and determine hospice appropriateness.
Educate patients and families about hospice philosophy, services, Medicare benefits, and what to expect .
Coordinate a seamless transition into hospice services.
Collaborate with physicians, hospital discharge planners, social workers, intake coordinators, and the interdisciplinary team .
Complete timely, accurate EMR documentation while ensuring regulatory compliance.
What You'll Bring
Active Minnesota Registered Nurse (RN) license.
Experience in hospice, oncology, ICU, palliative care, home health, emergency medicine, or other complex clinical settings .
Exceptional communication, assessment, and relationship-building skills .
Confidence discussing hospice care with patients, families, and referral partners.
Ability to work independently while managing a flexible field schedule.
Valid driver's license and reliable transportation.
