Job description
Pathways Home Health and Hospice in Sunnyvale seeks an experienced Hospice Admissions Visit Nurse (RN) to evaluate and admit patients to Hospice, document observations and interventions, and coordinate care with the Case Manager and Clinical Manager.
California RN license and CPR certification are required; a valid California driver's license with auto insurance is required; shifts include Sun-Thu or Tue-Sat, 40 hours per week, with a $9,000 signing bonus (conditions apply).
