Job description
Haven Hospice in Palatka, FL is seeking a Registered Nurse to join our admissions team. You will visit patients in homes or facilities, discuss goals of care, collect medical histories, and determine eligibility for hospice services based on Medicare guidelines.
This role offers flexible scheduling within a 9a-6p/10a-7p/11-8 framework, teamwork with an interdisciplinary group, and opportunities for professional growth as part of a supportive, patient-centered organization.
