Job description
Chapters Health System is seeking an Admission RN to assess patients in their residence or hospital settings and initiate care within our services. This role acts as the ambassador of the organization, ensuring smooth transitions to Care Navigation.
The position requires an active RN license in the working state, at least 1 year of nursing experience (hospice/hospital preferred), and EMR/EHR proficiency. BLS certification and a valid driver’s license are required, with bilingual Spanish a plus.
