Job description
Seaport Hospice is seeking a compassionate Admission Registered Nurse (RN) to conduct comprehensive hospice admissions in patients’ homes in San Diego, CA. You will develop individualized care plans, coordinate with physicians and teams, and educate families on care options and disease progression.
The role requires a California RN license, 2+ years of RN experience (hospice/home health preferred), CPR/BLS certification, and valid driver’s license with reliable transportation.
