Job description
HopeHealth is seeking a Hospice Admissions Registered Nurse (RN) to assess patients, admit eligible individuals to hospice, and coordinate care plans. The role emphasizes family-centered, end-of-life care with a 40-hour week schedule from 1:00pm to 9:30pm and rotating weekend commitments. A $2000 sign-on bonus is offered.
The RN will ensure compliant admissions, participate in IDG meetings, and maintain EMR documentation while collaborating with payers and referral sources.
